GAME SPEED PERFORMANCE
PARENT/GUARDIAN WAIVER & RELEASE OF LIABILITY
This Waiver & Release of Liability (“Agreement”) is made between Game Speed Performance (“GSP”) and the undersigned parent/guardian (“Parent/Guardian”) of the minor participant named below (“Participant”). By signing this Agreement, Parent/Guardian agrees as follows:
1. Assumption of Risk
I understand that speed, agility, strength, and sports performance training involve physical activity, including running, jumping, cutting, sprinting, change of direction, and use of training equipment. I understand that these activities involve inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, overuse injuries, and in rare cases, serious injury or death. I voluntarily allow my child to participate in training with GSP and assume all risks, known and unknown, associated with participation.
2. Release of Liability & Indemnification
To the fullest extent permitted by law, I, on behalf of myself, my child, and our heirs, executors, and assigns, release, waive, and discharge Game Speed Performance, its owners, coaches, employees, contractors, and agents from any and all claims, demands, causes of action, or liability arising out of or related to my child’s participation in training, including any injury, illness, or property damage, whether caused by ordinary negligence or otherwise. I agree to indemnify and hold harmless GSP and its staff from any claims brought by or on behalf of my child or any third party arising from my child’s participation.
3. Health & Medical Authorization
I certify that my child is physically able to participate in athletic training and has no medical condition that would make participation unsafe. I agree that it is my responsibility to consult with a physician about my child’s ability to participate. In the event of injury or medical emergency, I authorize GSP staff to seek emergency medical care for my child, including transportation by emergency personnel, and I agree to be responsible for all related costs.
4. Photo & Video Consent
I understand that GSP may occasionally take photos or videos during training sessions for coaching review, social media, website, or other promotional use.
5. Policies & Expectations
I understand and agree that:
- My child must follow all instructions from coaches and staff for safety.
- I am responsible for ensuring my child arrives on time, properly hydrated, and wearing appropriate athletic clothing and footwear.
- Any scheduling, cancellation, or payment policies provided by GSP apply to all sessions.
6. Acknowledgment & Consent
I have read this Agreement, understand its terms, and sign it freely and voluntarily. I understand that by signing, I am giving up certain legal rights for myself and my child, including the right to sue GSP for ordinary negligence.
